Choosing a nursing home for a loved one is a decision rooted in hope—hope for safety, quality  care, and dignity. Unfortunately, nursing home neglect remains a serious and widespread  problem. Because neglect often begins subtly, many families don’t recognize the signs until it  has caused significant harm. Knowing what to look for is essential for protecting vulnerable  loved ones.

Below, our friends at Ganderton Law, LLC explain common warning signs of nursing home neglect.

Unexplained Physical Injuries

Bruises, cuts, fractures, or repeated falls are serious red flags. While minor injuries can occur in  any setting, consistent or unexplained injuries may signal:

  • Poor supervision
  • Unsafe conditions
  • Rough handling
  • Understaffing
  • Neglect during transfers

If a facility cannot clearly explain how injuries occurred, that’s cause for concern.

Poor Hygiene and Unclean Conditions

A resident’s hygiene and environment reflect the care they are receiving. Warning signs include:

  • Dirty clothing or bedding
  • Strong odors
  • Unclean bathrooms
  • Matted hair or unwashed skin
  • Insects, spills, or general mess in living areas

These signs may point to insufficient staff, failure to assist with daily tasks, or inadequate  cleaning routines.

Bedsores (Pressure Ulcers)

A personal injury lawyer knows that bedsores are one of the most alarming indicators of neglect. They form when immobile residents  are not repositioned regularly. Pressure ulcers are preventable with proper care.

Stages of bedsores can include:

  • Redness or irritation
  • Open wounds
  • Deep tissue damage
  • Infection

Bedsores often signal that a resident is spending long periods unattended, which is unsafe and  unacceptable.

Sudden Emotional or Behavioral Changes

Neglect affects more than physical health. Emotional red flags include:

  • Depression or withdrawal
  • Anxiety
  • Irritability
  • Fear of certain staff
  • Sudden personality changes
  • Refusing to participate in activities they once enjoyed

These shifts may reflect mistreatment, isolation, or inadequate stimulation and social support.

Weight Loss, Malnutrition, and Dehydration

Nutritional neglect is extremely serious. Warning signs include:

  • Rapid or unexplained weight loss
  • Fatigue
  • Dry skin or cracked lips
  • Sunken eyes
  • Repeated urinary tract infections (a sign of dehydration)

Residents rely on staff for meals, hydration, and monitoring food intake. Lack of proper nutrition  can be life-threatening.

Medication Issues

Medication errors are common indicators of neglect and include:

  • Missed doses
  • Incorrect dosing
  • Overmedicating or sedating residents
  • Failure to monitor side effects

Residents with chronic conditions may deteriorate quickly when medications are not  administered properly.

Staffing Problems

Neglect often stems from systemic issues. Warning signs include:

  • High staff turnover
  • Not enough caregivers on duty
  • Call lights going unanswered
  • Residents left waiting long periods for help
  • Staff appearing rushed, stressed, or dismissive

Understaffing is one of the strongest predictors of poor care.

What Families Can Do

Families should:

  • Visit frequently and at different times
  • Document concerns with dates, photos, and observations
  • Ask staff direct questions
  • Request care plans and records
  • Speak with supervisors or administrators
  • Trust their instincts

Early vigilance can prevent neglect from escalating.

Nursing home neglect is deeply harmful, but it is often avoidable. Recognizing the warning signs  empowers families to advocate for vulnerable loved ones and ensure they receive the  compassionate care they deserve.
