As of April 24, 2024 Glen Brook Rehabilitation and Healthcare Center has spent 30 months as an SFF, and their most recent inspection was in December of 2023.

The Special Focus Facility (SFF) Program, managed by the Center for Clinical Standards and Quality/Quality Safety & Oversight Group, identifies nursing homes that have a history of poor performance and have failed to meet certain health and safety standards.

This list indicates that Glen Brook Rehabilitation and Healthcare Center has been under increased scrutiny due to ongoing issues in areas like care quality, safety, and compliance with regulatory requirements. The facility, located on 801 East 16th Street Berwick PA, has not met specific standards, highlighting concerns about its ability to provide safe, effective care to residents. 

Glen Brook has faced multiple inspections revealing deficiencies that resulted in actual harm to residents. This includes issues such as inadequate staffing, poor care practices, and failure to meet required safety standards. The facility has received 1-star ratings for overall care, health inspections, and staffing, which are the lowest possible ratings from the Centers for Medicare and Medicaid Services (CMS).

Although it has excellent weekend staffing, the nurse staffing is still below the state and national norms.

Additionally, Glen Brook has faced multiple health violations in its most recent inspection and has paid 20 fines in the last three years totaling over $373,000. These factors suggest that the facility may not provide the highest level of care and safety.

This amount of staffing is considered unsafe and has been linked to poor care and increased risk of injury for residents. Furthermore, Glen Brook has faced a significant number of days with inadequate staffing, with 79.7% of days in 2022 falling below the minimum safe staffing level.

The facility’s placement on both the NHAA Watchlist and the SFF is cause for serious concern.
